DPW Revises Forms for Personal Care Home Support Plan

On January 22 the PA Department of Public Welfare (DPW) announced that in response to provider requests, edits have been made to the Personal Care Home Support Plan form to include questions concerning the resident’s receipt of the plan. The form now includes two questions: (1) Does the resident (or designee) request a copy of the plan? and (2) Is a copy of plan given to the resident or designated person upon request? DPW says that use of the new version that includes these questions is not required. DPW also says that use of the old version of the form is not a regulatory violation.
